This is the second form in the special event rental process and should only be filled out after your date has been approved.

Wedding Calendar Request

Request a date to hold a wedding at Grace Church of Mentor. All weddings are subject to pastoral and office approval and must be scheduled at least 8 weeks prior to the desired date.
